Enter your first and last name as it is on file with the Social Security Administration (SSA).  This information will be verified with the SSA.  If the information does not match what is on file with the SSA, we will not be able to finalize your claim.  If you have worked under any other last names, it is important we know that both for the SSA verification and also to make sure we get all your wages from the wage file.
