Validate Existing UI Account

For your protection, the Indiana Department of Workforce Development is asking questions that will help authenticate your identity.  The questions on this screen ask for information that is unique to your company and only known by responsible parties within your company.  

Your Federal Employer Identification Number (FEIN) is the number assigned by the IRS and can be found on your Federal Form 940 - FUTA Tax Return and Form 941- Quarterly Tax Return.

Your SUTA number is your Indiana unemployment tax account number.  It can be found on your UC-1, Indiana Quarterly Contribution Report.

Also please provide the Total Gross Wages reported on your most recent UC-1, Indiana Quarterly Contribution Report.
