Federal Certification Request

The Federal Certification Request page is used to request a certification or recertification to the IRS of the SUTA wages and contributions you have paid to DWD.  To request a certification:

• Select a year from the dropdown box

• Enter the reason for the request in the” Request Comments” section

• Click the checkbox that confirms your authority to request the certification and affirming that the information provided is true and correct.

• Click the “Submit Request” button

DWD will generate and mail a copy of the certification to you and the IRS.
