Required Information

Be prepared to provide the following information about your business or organization when you begin the registration process:

 

  • Federal Employer Identification Number (FEIN). A link is provided to the IRS to get an FEIN if you do not yet have one.

 

  • Name of legal entity (40 characters maximum)

 

  • Doing Business Name (DBA), if it is different than the legal name (40 characters maximum)

 

  • The physical street address of the business (40 characters maximum)

 

  • Mailing address (if different from the physical address) (40 characters maximum)

 

  • Additional business location addresses for your business

 

  • Type of legal entity (corporation, sole proprietorship, LLC, etc.)

 

  • Primary activity your business performs

 

  • If a corporation or partnership: state and date of incorporation or formation

 

  • Name, social security number, telephone number for owner(s)/officers/partners of your business

 

  • Dates that covered employees first performed services within Indiana

 

  • Date that first wages were paid to covered employees

 

  • Payroll service/agent name (if applicable) and type of correspondence you want them to receive

 

If you start your registration and discover you do not have the information needed to complete the process, you may exit the application and return to where you left off if you log back on within 30 days.
