Legal Information

To register your business with DWD, you need to provide the Legal Business Name of your business.   If you are a sole proprietor, the legal name is your legal name, e.g. John Smith.  Corporations, partnerships, governmental entities (other than federal agencies), not-for-profit organizations, and associations must provide the legal name provided to the IRS when you registered for your Employer Identification Number (FEIN).  The maximum number of characters for this field is 44.  

If you have an alternate name or business name for your business (dba), please enter that name in the “Doing Business As:” field.  You may provide multiple dba names.   The maximum number of characters in this field is also 44.

Select the Business Type that pertains to your business.  Only one business type may be selected.  The business type selected must agree with the information filed with the IRS in the request for the FEIN.

If you are corporation or partnership, please enter the date of the Formation Date of Corporation or Partnership as well as the State of Incorporation or Formation.  The date must be entered in the format mm/dd/yyyy or you may select the date from the calendar icon.  Please select the state from the dropdown box list.

All businesses must enter the Date Payroll Began in Indiana.  The date must be entered in the format mm/dd/yyyy or you may select the date from the calendar icon.

Primary Address

The primary address is the address all of your Unemployment Insurance forms will be sent to unless you indicate otherwise on the subsequent “Correspondence Agent” screen.

• Address Line 1 is for your physical street address, e.g. 101 W Main Street

• Address Line 2 can be for additional address information, e.g. Suite # or post office box

• City is the city the business is located

• State can be selected from the dropdown box listing.  

• Zip/Postal code, including the four digit extension if known, if the business is located in the US or Canada

• If your business is not located in the U.S. or Canada, enter your province name in the “Non-USA State Province” field.

• Enter the business’s telephone and fax numbers in the format 999-999-9999.

Update Employer Legal Information

You may update your Primary Mailing Address and Telephone/Fax Numbers from this page. Type in the correct information and click “Update.”  You will receive a confirmation that the legal information has been successfully updated.  After you have received the confirmation you may Logoff or make another selection from the menu.
